Minnesota Hockey Waiver Form

Waivers must follow Minnesota Hockey's "Participation Rule" per the Minnesota Hockey Handbook, Section IV. The form below is "editable" and must follow the below procedure:

• Waiver form to be initiated by the parent(s) who must fill out Parent Player section. The form is then forwarded to the "releasing" Association President.
• Releasing Association President then signs if approved (adds conditions if any) and forwards waiver to the receiving Association President.
• Receiving Association President sins, if approved and then forwards to District 11 (Pete Hill & Kris Bryant) for District 11 approval.
• District 11 signs waivers and then sends back to all parties for their initials on the "conditions" of the waiver if any.
• Final approved copies will be emailed to all parties.
